Welcome to the 2008 Putt-Putt Tournament information and golfer
registration page! This year's event is presented by the Kingsport
Evening Civitan Club.
All ages are encouraged to join the fun and help chronic kidney patients in East Tennessee. This is a great team-building exercise for businesses or for family fun!
Below is listed all the information on this year's event. Simply
fill-out the registration form, choose your method of payment, and you will be automatically registered
for the event.
General Information
- Date: Thursday, May 15, 2008
- Time: Registration and practice begins at 5:00 pm; tournament begins at 6:00 pm
- Place: Putt-Putt Golf Course, 346 W. Stone Drive, Kingsport, TN
- Format: 36-hole best-ball tournament with teams of four
competing for prizes and trophies! There are also exciting challenge
holes and door prizes. There will also be food available with
the proceeds also benefiting the tournament.
- Entry Fee: $25/player, $100/team
- Hole Signs: $50 each
- Last date to register online: May 9
2-Step Registration Process
STEP 1: Submit your player information.
In the email form below, fill-out all the information necessary
to register yourself as well as the rest of your team. If
you do not have a complete foursome, please check 'please assign
a player' for all open spots and we will attempt to fill those spots
for you. Review your information carefully, then click the "Submit
my info" button. After you are redirected to the "Thanks!
Your response has been received" page, click "Back"
and proceed to:
STEP 2: Submit your credit card information or other form of payment.
